Consider using another door, buy a grab rail, repair or replace the door threshold, call CSC for a further conversation - you may need to replace the door with a level threshold door, or a threshold ramp.

I am having difficulties using the path and worried about falls 

Consider clearing bushes, replacing broken slabs or paving, try a walking stick, use an alternative entrance, replace the path, widen the path, fit a handrail along the path, alter the gradient of the path. Call CSC for a further conversation.

I am having difficulties using the steps at the property, there are too many or they are too deep / shallow

I want to be able to get in and out of my property safely

Move furniture to create more space, adjust door furniture/tension in door spring, buy a key grip. More complex issues could be solved with a door opening system - this would require an OT assessment.
