Neighbourhood Team

Referral criteria:
-You have exhausted all options and need help with an issue/problem and there are still unmet needs.
-The person is complex in terms of their circumstances:physical/mental health,social and financial.
-The person may be at risk of admission/readmission to hospital.
-The person is a frequent user of services.
-The person has had a significant deterioration in their health and wellbeing.
