NHS Diabetes Prevention Programme (NDPP)

The NHS Diabetes Prevention Programme (NDPP) is a Free of Charge programme of education for those at high risk of developing Type 2 Diabetes. It is a joint commitment from NHS England, Public Health England and Diabetes UK. The programme has been available in Lincolnshire since 2016 and is provided by Xyla. It empowers participants to make the small sustainable changes to their own unique lives and lifestyles required to reduce the risk of becoming Type 2 Diabetic. Eligible patients with an HbA1c of between 42 mmol/mol and 47 mmol/mol, performed within the previous 12 months can either be referred by their Health Care Professional at their local GP Practice or they can self-refer with this information plus their NHS Number by following this link: Register for the NHS Diabetes Prevention Programme or call: 0333 577 3010.
Women with a history of Gestational Diabetes can also self-refer without the need of a current blood test by simply stating their history of Gestational Diabetes by following this link: https://preventing-diabetes.co.uk/gestational-diabetes or call: 0333 577 3010.
