Welcome to Unirose, a unique Care Coordination and Companionship service that provides people with support in a range of ways in real-time:
- helping people to navigate health and social care pathways, accompanying to any appointments, exploring finding routes, regular visits, etc
- help or lead on exploring new care packages options and/or monitoring the quality of existing care support
- companionship, shopping, outings, holidays
- supporting people to get home from care/nursing homes or transitioning into 24/7 care
- completion of Attendance Allowance, PIP forms or similar
- future planning, including Lasting Power of Attorney or end of life support
- helping to access community groups or social activities/events
- out of hours access to the team for reduce carer crisis or to help deal with difficult situations
