Below you will find a number of dementia and memory support services to support you and your brain health.
Local Support Services and Resources
Provided by Lincolnshire Partnership Foundation Trust (LPFT). The service gives you access to a support worker who will provide guidance and advice throughout all stages of your dementia journey. It also accepts referrals from the public and professionals in health and social care (for example, general practitioners (GPs)). Tel: 03031 234000
I Forget Service - Age UK Lincoln and South Lincolnshire
For those experiencing symptoms of memory loss and cognitive impairment with or without a diagnosis of dementia. Their friendly and knowledgeable advisors are on hand to provide a listening ear, clear advice, and kind support, throughout your journey with dementia. LPFT - Mental health and learning disabilities services
Lincolnshire Partnership NHS Foundation Trust (LPFT) provide a range of mental health services, as well as support for people with learning disabilities. Single Point of Access. Telephone: 0303 123 4000
Provides emotional wellbeing support to children and young people up to 19 years old (25 if special educational needs/disability or leaving care). They can help with a range of emotional wellbeing concerns e.g. exam stress, worries, low mood, low body image, self-harm, relationship difficulties, low self-confidence. Countywide service supporting adults across Lincolnshire to achieve confident, fulfilled, and independent living. Access to the service is subject to basic eligibility criteria. To make a referral to the Wellbeing Service, call 01522 782140

They offer services to support you at every stage of your caring journey to help you find the balance in your life you wish to achieve. The customer service centre provides telephone-based information and support around your caring role, and Carers First work directly with carers in local communities across Lincolnshire so you can live your life to the fullest. Blood Pressure Checks
If you are over 40, you can get your blood pressure checked for free at most pharmacies across Lincolnshire. Your pharmacist will refer you to a GP if your blood pressure is higher than normal and can offer advice on how to manage it.
Many pharmacies are open evenings or weekends, so why not pop in and ask about getting your blood pressure checked next time you pass? If your local pharmacy doesn’t offer checks, they will give you information about a pharmacy that does.

Hearing and Vision Tests
Audiology - United Lincolnshire Hospitals. Audiologists test patients’ hearing during clinics, they also perform diagnostic tests requested by medical staff and complex hearing tests.
